Covid -19 Self Declaration Form

Each participant will be required to fill out a mandatory Covid -19 self declaration form for facility / on ice program access.

Without a declaration  – access is not allowed.

These forms will be collected by Candace or Marcela of Tucker Hockey on the first evening. If there are any changes to the status of the answers submitted on the first day it is the responsibility of the participant to email programs@tuckerhockey.com ASAP to inform us of these changes. We will place a credit on file for any missed sessions due to COVID-19 related symptoms.
